We are excited to introduce the Heart Failure Care Map, a resource to support provider’s to navigate individuals with heart failure to local resources.

Care Map Key Features:

Diagram: Provides an overview of Heart Failure diagnosis and management, including information to assist clinicians to determine when to make a referral.
Provider Resources: A small, curated list of relevant clinician-facing resources.
Patient Resources: Includes hand-outs, educational videos, and programs that patients can self-refer to.
Referral by Local Health Hub: Inventory of releant programs, broken down by Local Health Hub.
